TWIN LAKES ELEMENTARY BACKGROUND CHECK REQUEST Form
This is NOT the background check but a REQUEST form to have one sent to you via email. It can take a few days for the email to arrive.
Please complete ALL sections with THE CORRECT SPELLING and information requested!
An email will be sent to the address you provided to complete your background check. It will come from ISD 728-Volunteers with a link to TRUSTED EMPLOYEES. It sometimes ends up in your spam/junk mail folder. There will be a $10.25 processing fee.
If you had a background check done last year you do not need a new one this year.
